*Certain restrictions and exclusions apply. Referred patient must complete a physical therapy, occupational therapy or home health evaluations to qualify. Gift will be sent based on contact information retained at referral or to the most current address available in our system. Joyner Therapy Services will not issue multiple gifts in the event they are lost, stolen or otherwise unaccounted for. Type, size and vendor of gift will be chosen at random based on availability.
